Please print and fill out all applicable paperwork and bring it with you to our first session. Adults and couples will need to complete the Agreement, Adult Intake, HIPPA, and Release Form. Adolescents will need to complete the Agreement, Adolescent Intake, HIPPA, and Release Form.
